PATHWAYS: Health System Navigation
Why "pathways": The wavy lines in our logo are a symbol for the many journeys we are on as trans people, gender diverse people, and/or as people who have unanswered questions about our gender identities. You may be charting a course that is a straight line; it can also loop, double-back, experiment, or drift. Often we wish to access health care and social supports along our personal journey. We see health system navigation as a service that can help pave paths to help you on your way.
Our health system navigator has two main tasks:
1. Connect you to the care you need as a trans or gender diverse person;
2. Help you overcome logistical and information barriers to healthcare options.
The health system navigator also works with others to create trans-knowledgeable services and improve care pathways for trans and gender diverse people.
